COVID-19 Liability Form

The COVID-19 Liability Form is required if you are attending an in-person event, Kiwanis sponsored event.
You must complete the following steps:

Download and print out the liability form

Sign the form physically, then scan the form (Hint: You can use the Notes app on your phone. Camera icon → "Scan Documents")

Email the form to Ella (ellatang329@gmail.com) with the subject line "Oakland [YOUR FULL NAME] [EVENT NAME]"

Bring the signed form to the event - If you don't bring it, you will not be allowed to attend the event!
Medical Form
Medical Forms are needed/required to attend events such as DCON, FRN, RTC, and certain DCM's. If you do not have a printer to complete your medical form, please talk to an officer to let us know.
Attendance/Event Request Form
If you are attending another division's DCM or event, you must fill out an Attendance Request Form in order to be able to attend the event. If you need help filling it out, contact us and we'll be more than happy to help.
